Stress Fracture vs Shin Splints: How to Tell the Difference
If you run regularly, there's a good chance you've had sore shins at some point.
Sometimes it's nothing more than an overloaded muscle or irritated bone. Other times, it's the start of a stress fracture.
The tricky part is that they can feel very similar early on. Knowing the difference can help you make the right decision before the injury becomes more serious.
What are shin splints?
Shin splints, also known as medial tibial stress syndrome (MTSS), are one of the most common running injuries. They usually develop when the muscles and tissues attached to the shin bone become overloaded. This often happens after increasing training, changing shoes, returning from a break or running more hills than usual. The pain is typically spread out along the inside border of the shin rather than one exact spot. Many runners notice it during the first few minutes of a run before it settles, only to return afterwards.
What is a stress fracture?
A stress fracture is different.
Instead of the surrounding tissues becoming irritated, the bone itself begins to develop tiny cracks from repeated loading. It doesn't usually happen after one run. It builds gradually when the bone isn't able to recover between training sessions. Stress fractures most commonly affect the tibia (shin bone), but can also occur in the foot, pelvis and femur. Unlike shin splints, continuing to train through a stress fracture can turn a small injury into one that requires months away from running.
Four signs it could be a stress fracture:
1. You can point to the pain with one finger
Stress fractures usually become very localised.
If you can place one fingertip directly over the sore spot, it raises our suspicion.
Shin splints are more commonly felt over a broader area.
2. The pain is getting worse, not better
Most overload injuries improve when training is adjusted.
Stress fractures often do the opposite.
The pain gradually becomes more frequent, more intense and starts appearing earlier during your runs.
3. It still hurts after you've stopped running
Pain that lingers for hours after a run, or is still noticeable the next morning, deserves attention.
This doesn't automatically mean it's a stress fracture, but it shouldn't be ignored.
4. Walking or stairs are starting to hurt
This is one of the biggest warning signs.
If everyday activities are becoming painful, it's time to stop running and get assessed.
By this stage, the bone may no longer be tolerating normal daily loads.
So when should you stop running?
If the pain is becoming more localised, getting progressively worse or affecting walking, don't try to push through it. The earlier a stress fracture is identified, the sooner the right management can begin. Waiting another week or two in the hope that it settles often means a longer recovery.
Not every sore shin needs a scan, but persistent symptoms are worth having assessed by someone experienced with running injuries.
